Dr. Firdaus Delivers In-Depth Men’s Health Seminar at Proton Man’s Health Day 2025

He explained that BPH occurs due to “a swelling of the prostate gland,” which leads to issues like difficulty passing urine, poor urinary stream, repeated trips to the toilet, and nocturnal urination (nocturia). These symptoms, he warned, often disrupt daily routines and significantly affect sleep quality. For working men, especially in manufacturing sectors, these issues can easily go unnoticed until they escalate.


Early Detection Through Proper Screening

The seminar went deeper into how prostate-related conditions are diagnosed. Dr. Firdaus explained the use of the International Prostate Symptom Score (IPSS) to determine severity, adding, “The severity of BPH does not depend on the size of the prostate. It depends on the symptoms of the patient.”

He also outlined the importance of PSA (Prostate Specific Antigen) blood tests, stating, “When the PSA is high, it reflects the size of the prostate… and helps us assess potential risk of malignancy.

He clarified that imaging procedures such as ultrasound are not performed merely out of routine but play a critical role in detecting abnormalities: “Through imaging, we can estimate the size of the prostate gland and investigate the potential risk of malignancy.

For many employees in the audience—particularly those aged 40 and above—this was their first time hearing such detailed medical explanations.


Treatment Options: From Lifestyle Changes to Medical Intervention

According to Dr. Firdaus, the treatment pathway for BPH and prostate issues generally follows three fundamental components:

  1. Lifestyle Modification
  2. Medical (Pharmacological) Therapy
  3. Minimally Invasive Procedures

He emphasized that lifestyle remains the foundation of all treatment: “Lifestyle modification for any disease is the foundation for all treatments.” He explained how excessive sugar or carbohydrate intake increases insulin levels, and “when the insulin is high, the prostate gland will be easier to expand.”

He also spoke about the realities of modern working life:
“We go to work early, we don’t want to jam, we go home late… so we don’t have time to exercise. But is it fair for us not to exercise at all?”
This reminder encouraged employees to reflect on their own daily habits.

When lifestyle changes are insufficient, medications such as alpha blockers and 5-alpha reductase inhibitors may be prescribed. However, Dr. Firdaus cautioned about potential side effects:
“When we block the conversion of testosterone… the libido will be reduced.”

He urged participants to fully understand their treatment options and discuss them carefully with their healthcare providers.


Modern Minimally Invasive Procedures

The seminar also highlighted advancements in minimally invasive treatments for prostate enlargement.

TURP – Transurethral Resection of Prostate

Dr. Firdaus described TURP as the current gold standard:
“We will cut all the excessive prosthetic tissue in the urethra… to open the passage.”

Rezum / Water Vapor Therapy

He introduced a newer, less invasive technique that uses heated steam to shrink prostate tissue:
“It will insert a very small needle… inject steam… and the tissue will explode and form a scar. When there is a scar, it shrinks, and the channel will reappear.”

He noted, however, that the treatment may need to be repeated in some cases, as results can vary.


Prostate Cancer: Understanding the Risks and Outlook

One of the most impactful segments of the seminar focused on prostate cancer. Dr. Firdaus explained that PSA screening and digital rectal examination remain essential tools for early detection.
“When the PSA is high… we have to do a biopsy. The biopsy will tell us whether the cancer is high risk, low risk, or moderate risk.”

He reassured the audience that prostate cancer is generally slow-growing:
“Overall survival is about 87% compared to 5 years after detection.”

Because of this, some patients may opt for active surveillance rather than immediate treatment, depending on their risk level. He reinforced that healthy lifestyle habits—especially weight management and balanced nutrition—play a major preventive role.
